How to Apply for Convalescent Scheme, Grants, and Health & Safety Courses
Convalescent Scheme Application Process
To apply for our convalescent scheme, please follow the steps below:
Page one: Please complete all questions and sign/date the form.
Page two: Clients are asked to get their current or former employer or Trade Union to confirm that they have worked in the Cotton textile Industry. If you are unable to do this then the Trust Secretary will assist you in obtaining a relevant signature to act as the sponsor.
Page Three: The form has to be filled in by the clients Doctor who must state that you as a client would benefit from a period of convalescence and that you do not require nursing assistance as the convalescence is provided at a normal commercial Hotel in Blackpool. (Most doctors currently charge for this service which the client must pay for ) This will be reimbursed if your application is successful/
Page Four: contains the rules of the convalescence scheme along with the address of the H.M. R. C. dept who can provide you with confirmation of all your employment history (thus confirming your employment within the Cotton Textile Industry within the last 40 years)
